Patient Representation Group

Dear Patient

We would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we are setting up a virtual patient representation group so that you can have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

If you are happy for us to contact you occasionally by email, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

Patient Questionnaire - How is the Practice Performing?

To find our how the Practice is performing and to review the opinions and feelings of our patients High Green Medical Practice has in the past few weeks distributed copies of the Practice questionnaire to patients attending the Practice. Questionnaires have also been emailed or posted to members of the Practice Patients Participation Group. The Questionnaire was also posted on the Practice website for visitors to complete.

Action Plan

The Practice has reviewed the findings of the survey and has developed an Action Plan in response to the main areas of concern highlighted by respondents of the questionnaire. Copies of the action plan will be issued to members of the PPG by email and post and will be available within the Practice for review by all patients. As a Practice we would like Patients to review the proposed action plan and feedback their thought to the Practice. Within the Practice patients are asked to fill out one of the comments forms available and post their responses in the Practice Suggestion Box. Member of the PPG and visitors to the Practice website are asked email the Practice their thoughts using the following email address: We want to hear everyone's views.

Coffee & Cake Event - 19th February 2014

The Practice is also organising a coffee & cake event at the Practice on the 19th February 2014 between 11.30 & 13.30 this will be an informal drop in event open to all patients where patients can meet the GP's and Practice staff and discuss their thoughts regarding the Practice and also the proposed action plan. We want to see as many patients and members of the Patient Participation Group as possible at the event. If you are unable to attend we still want to hear your views so please either email us or fill out a comments slip.
